The facility JSON comes as one file per state because the national file would pass 20 MB. Each state file has the same columns as the national CSV. manifest.json lists every file with its row count, size, key and the fields we track for changes, so a script can fetch the lot.

Where it comes from

Nothing here is scraped or estimated. The facility file joins five CMS files (provider information, ownership, Medicare enrollment owners, enrollments, changes of ownership) and HUD's healthcare endorsement database and insured-mortgage files. HUD has no CCN, so the loan columns rest on our name, city and bed-count match; the methodology explains it and lists what it can get wrong. Primary sources: CMS Provider Information and HUD FHA insured mortgages.

The files are rebuilt from the publishers' newest copies each time the site's data refreshes. CMS posts monthly; HUD's mortgage files are monthly and its endorsement database quarterly. The file names carry the as-of date.

Nursing home columns

Blank means CMS or HUD left it empty. Dates are YYYY-MM-DD. Names stay in the capitals CMS uses. Columns marked "tracked" are the ones a facility alert watches.

`ccn`	CMS certification number (six characters, keep as text). The key.
`name`	Provider name as CMS publishes it.
`legal_name`tracked	Legal business name of the licensed operator.
`address`	Street address.
`city`	City.
`state`	Two-letter state.
`zip`	ZIP code (text).
`county`	County.
`ownership_type`	CMS ownership type, e.g. For profit - Corporation.
`certified_beds`	Number of certified beds.
`avg_residents`	Average residents per day.
`occupancy_pct`	Average residents divided by certified beds, percent.
`provider_type`	Medicare, Medicaid or both.
`in_hospital`	Located inside a hospital (true/false).
`ccrc`	Part of a continuing care retirement community (true/false).
`certified_since`	Date first certified for Medicare or Medicaid.
`chain`tracked	Chain or affiliated entity CMS names, if any.
`chain_id`	CMS chain ID.
`chain_facilities`	Facilities in that chain.
`star_overall`tracked	CMS overall rating, 1 to 5.
`star_health`	Health inspection rating.
`star_quality`	Quality measures rating.
`star_staffing`	Staffing rating.
`special_focus`tracked	SFF or SFF Candidate, blank if neither.
`abuse_icon`	CMS abuse icon (true/false).
`nurse_hprd`	Total nurse staffing hours per resident per day (PBJ).
`rn_hprd`	RN hours per resident per day.
`weekend_nurse_hprd`	Weekend total nurse hours per resident per day.
`nurse_turnover_pct`	Total nursing staff turnover, percent.
`rn_turnover_pct`	RN turnover, percent.
`administrator_departures`	Administrators who left in the CMS period.
`fines_count`	Fines CMS reports.
`fines_total_usd`	Total of those fines, dollars.
`payment_denials`	Payment denials CMS reports.
`last_survey`	Date of the latest standard health survey.
`health_deficiencies`	Health deficiencies cited on that survey.
`owners_direct`tracked	Direct owners with 5% or more (name and share), semicolon-separated.
`owners_indirect`tracked	Indirect owners with 5% or more, semicolon-separated.
`operators`tracked	People and organizations with operational or managerial control.
`mortgage_security_interest`tracked	Holders of a 5%+ mortgage or security interest reported to CMS.
`pe_owners`tracked	Owners the facility flagged as private equity in its Medicare enrollment.
`reit_owners`tracked	Owners flagged as a REIT in its Medicare enrollment.
`chow_count`tracked	Changes of ownership in the CMS file (January 2016 on).
`latest_chow_date`tracked	Effective date of the most recent change of ownership.
`latest_chow_buyer`	Buyer in that change.
`latest_chow_seller`	Seller in that change.
`hud_records`	HUD Section 232/242 records matched to this facility (fiscal 2001 on).
`hud_loan_status`tracked	active, terminated, unknown, or none.
`hud_fha_number`tracked	FHA project number of the active loan (or the latest record).
`hud_program`	HUD program of that loan.
`hud_endorsed`	Endorsement date of that loan.
`hud_original_amount`	Original insured amount, dollars.
`hud_rate`tracked	Note rate of the active loan, percent.
`hud_upb`tracked	Unpaid principal balance of the active loan, dollars.
`hud_monthly_pi`	Monthly principal and interest, dollars.
`hud_maturity`	Maturity date of the active loan.
`hud_servicer`tracked	Servicer of the active loan.

`program`	HUD program at endorsement.
`facility_type`	HUD facility type.
`endorsed`	Initial endorsement date.
`fiscal_year`	HUD fiscal year of endorsement.
`original_amount`	Insured amount at endorsement, dollars.
`lender_at_endorsement`	Lender named at endorsement.
`hud_beds`	Beds in HUD's record.
`match_score`	Name-match score from the site's matcher (higher is closer).
`status`tracked	active, terminated, or unknown (in neither HUD mortgage file).
`rate`tracked	Note rate, percent (active loans).
`upb`tracked	Unpaid principal balance, dollars (active).
`monthly_pi`	Monthly principal and interest, dollars (active).
`maturity`	Maturity date (active).
`term_months`	Loan term in months (active).
`holder`tracked	Holder (active).
`servicer`tracked	Servicer (active).
`terminated`tracked	Termination date (terminated loans).
`termination_type`tracked	How it ended, e.g. Prepayment.

Alerts on a facility

Every facility page has a "Watch this facility" box. When a monthly refresh shows a change in any tracked column for that CCN (owners, operators, lenders of record, a change of ownership, the overall star rating, Special Focus status, or the HUD loan's status, balance, rate or servicer), the alert fires. Alert emails have not started yet; sign-ups are saved and the first email will ask you to confirm. One thing to know: an active HUD loan's balance falls every month as it amortizes, so a facility with an active loan will show a balance change on most refreshes.

These answer with JSON and allow cross-origin requests.

The underlying CMS and HUD data is public domain. Our compilation (the join, the cleaned columns and the HUD match) is yours to use under Creative Commons Attribution 4.0: credit it as "Data: SNF Register (hudskillednursing.loan)" with a link to this page. The rate figures carry their publishers' terms: Treasury yields are public domain, and SOFR comes from the Federal Reserve Bank of New York under its terms of use. No warranty: matches can be wrong and files can be out of date. For a bulk feed, an API key or licensing on other terms, write to desk@janover.co.
